We are making some updates at Alterity!
Please take note of the following key dates and information:
🗓️ Key Dates:
- Starting December 1, 2024, our landlords will take over the common areas in our building.
- We’ll be closed from December 23rd to January 5th for these changes.
- We will reopen on January 6th with Dr. Burdge, Amy Epp, and Trina in their same spaces. You may notice some different signage and other changes in the building.
📄 New Patient Forms:
Beginning January 6th, updated consent, financial, and card-on-file authorization forms will be required to streamline billing and improve service.
